What is Elder
Case Management? 

Case management is an extremely helpful service for everyday life.  Our case manager will help you handle confusing tasks such as filling out paperwork, locating resources such as fuel assistance or transportation options, making phone calls to providers or insurance companies, scheduling appointments, and more.

Case Management is here to help the elderly and their caregivers relieve stress and enjoy the things that really matter during this transitional period.

Meet The Founder

arah Dionne

is a caregiver like you...

While caring for her aging parents, Sarah manages her businesses, cares for her 8-year-old daughter, supports her husband's career, and manages to get dinner on the table.

 

As her parents' needs grew, Sarah felt desperate for help.  She called everywhere to find an elder case manager and advocate, to her shock, she discovered...they didn't exist in an accessible range!  She could find this organizational, supportive service for the developmentally disabled and mentally ill...but not the elderly who need help just as much.

 

Sarah recognized this lapse in necessary support as a disservice to the elderly and their caregivers, she felt called to fill the gap.  

 

Local Elder Resources was born.  

 

Now this resource is available to you!  And we are honored to help.  For Sarah to continue expanding this service, she has to show the incredible need within the community.

 

Receiving our help will, in turn, help others along the same difficult and transitional journey also have access to the same support.

Sarah Dionne is founder of Local Elder Resources and Sarah Dionne & Associates, a psychotherapy practice.

Sarah is a seasoned psychotherapist, She founded Whole Health Collaborative in 2019, a supportive group of psychotherapists that work together to create thriving independent practices. Today WHC is a functioning entity of its own.

Sarah is also a spiritual guide and bereavement counselor, she is seminarian with the Interfaith Seminary at Tree Of Life Interfaith Temple in NH and will be ordained as an Interfaith Reverend in 2026. 

She is available for supportive spiritual services to guide you through hospice care and the inevitable death of an elder loved one.

Ginny cared for her husband for many years.  She was dedicated to his wellbeing until his passing in 2023.  Ginny understands the challenges of caregiving intimately.  Yet, now she continues to be a helpful and a present member of the community.
 
Ginny is savvy at finding resources and has been doing so for many years as she's cared for her husband and now offers  support to friends and family.  She has run local non-profits, helped organize events through the years, and continues to engage with the community as an aid to those who need her.
 
Ginny is prepared to be dedicated to locating and organizing your many needs.  She enjoys finding what is available and linking you to the services you require.  
 
Bringing Ginny into your circle of support is the first step to relieving stress and overwhelm during this challenging time.
